Our office opened in fall 2011 and we are accepting new patients.

You can pre-register as a new patient by clicking on the Medical Records/Patient Portal page and following the instructions given there. If you registered/made an appointment on the phone, please fill out the forms given below and bring it with you at your appointment. This will enable you to be seen on time.

If you are an exisiting patient you can make/reschedule appointments or send messages by logging in to the patient portal link given in the 'Medical Records' tab.

All prescription refill requests should be called in to the pharmacy first.
